School, Camp, & Daycare Forms

We realize forms are a part of being a parent—whether it be a sports physical, camp enrollment, school medication, or just a note to avoid dairy. Our goal is to make this process as smooth as possible, so you can focus on your child’s next adventure!

To avoid delays, please complete your portion of all forms prior to submitting to our clinic, including the patient questionnaire on the OSSAA sports physical. You may drop off, mail, or fax forms to our office, or upload them directly to the Patient Portal. Please allow 3-4 business days for form completion.

Please Note: All forms require a well visit completed within the last year. Due to the number of form requests our office receives and the time and resources they require, we do charge a form processing fee (outlined below).

Clinic Form Processing Fees

During Your Visit:

There are no fees when forms are presented and completed at a well check or a visit dedicated to the purpose of the forms (such as a sports physical visit or asthma recheck).

Outside Your Visit:

There is a $10 processing charge for any forms requested outside of a visit.

For Expedited Forms:

If forms are required to be completed within 24 hours of receipt, there is an additional $10 rush fee.

For Complex Forms:

For more time-consuming and complex documents, like FMLA or TEFRA, we charge a $20 processing fee.
