Joining the Parkside Pediatrics Family
To register your child as our patient, please contact our office directly. The Parkside Pediatrics team will be prepared to answer your questions, schedule your first visit, and discuss everything you need to establish care for your family.
New Patient Registration
On the day of your child’s first appointment, we ask that you come in early to complete your registration. For your convenience, you can download and print our New Patient Forms to complete before you arrive, or you can complete the paperwork at our office.
We recommend bringing the following to your first visit:
Insurance Information:
A copy of your insurance card so we can verify benefits, if applicable.
Medical Records:
A copy of your child’s health history.
Immunization Records:
Your child’s complete immunization record.
Photo ID:
A valid ID for the parent or legal guardian.
Your Questions:
A list of any specific concerns or transitions you’d like to discuss with us.

At Parkside Pediatrics, we value an unhurried approach to care, so we typically schedule some extra time for your first visit. Can I register all of my children at once?
Absolutely! We would love to care for all of your little ones. When you contact our office, just let us know how many children you are transferring, and we will help you coordinate the paperwork and initial appointments for everyone at the same time.
How do I transfer my child’s medical records?
Once you’ve established care with Parkside Pediatrics, our team can help facilitate the transfer of records from your previous pediatrician.
Can I schedule a sick visit as my child’s first appointment?
To provide the most comprehensive care, we prefer to establish a relationship through a wellness visit first. However, we know life happens. If your child has an urgent need, please call our office, and we will do our best to coordinate their onboarding while helping them feel their best again.
